I am in-network with Blue Cross Blue Shield (PPO and Blue Choice Plans), United Healthcare/Optum, and Aetna.

I will check your benefits prior to our session. Additionally, I encourage you to call the number on the back of your card to verify your benefit information.

• What is my in-network deductible for outpatient mental health visits?

• Do I have co-insurance for outpatient mental health professionals?

• How much of my deductible has been met and is coverage applicable before or after I meet my deductible?

Out-of-network insurance

For most other insurance plans, I am an out-of-network (OON) provider. To use your OON benefits, you would be responsible for full payment at the time of service. I will provide you with a superbill to submit to your insurance company for you to be reimbursed. To understand your benefits, please call the number on the back of your insurance card to ask about out-of-network mental health coverage for CPT code 90837.

Some additional questions to ask would be:

• Do I need a referral from my primary care physician to a mental health professional?

• Do I need pre-approval from the insurance company prior to seeing a mental health professional?

• What is my out-of-network deductible for outpatient mental health visits and must this be met before receiving reimbursement?
